Step 2: Complete the APD Application


You can find the application on the APD website (apd.myflorida.com). The form will ask for personal details, medical history, and information about the individual’s daily functioning. Fill it out as completely as possible.


Step 3: Submit the Application


You can submit the application:


Online

By mail to your local APD office

In person for hands-on assistance


Step 4: APD Review and Assessment


Once submitted, APD will:

• Determine if the applicant meets eligibility criteria


Step 5: Approval & Next Steps
